Facility Evaluation Report
On/10/22/25 at 11:00 am Licensing Program Analyst (LPA) Lorena Casillas met with Licensee Byranisha Goodwin for an unannounced one (1) year required visit for this facility. LPA was greeted and granted access by Licensee. LPA explained the reason for the visit and an entrance interview was conducted.
Licensee informed LPA that there are no residents and that Licensee is ready to close the facility, forfeiting the license. Licensee is no longer interested in keeping the facility open and wishes to cease operation. Licensee states that the home will be used as a primary residence instead of a facility. Licensee handed LPA letter wishing for facility to be closed.
A tour of the physical plant was conducted with the Licensee at 11:10 am. The facility has three (3) bedrooms and two (2) bathrooms. Facility is currently not occupied by any residents. Licensee is the only person living in the home. Facility has been absent from residents for over a year.
LPA collected the license during the visit. LPA confirmed mailing address to remain as the facility address. Licensee was informed that Community Care Licensing will be mailing an official closure letter. LPA reminded the Licensee/Administrator that if he or she should decide to reopen, then a new application with all the appropriate documents and fees shall be submitted to CCL and license must be approved before operation. LPA will send closure survey to Licensee by email to complete.
Exit interview conducted. Copy of report provided to Licensee.
