The inspector’s account

On 9/10/2025 at approximately 9:00 AM, Licensing Program Analyst (LPA), Angelica Segovia conducted an unannounced annual visit to the facility. LPA was greeted by the Program Director, Juan Carlos Guillen and stated the reason for their visit. The Administrator/Director, Cynthia Fernandez arrived shortly after to assist with today’s meeting.

LPA asked for Census, Staff and Client Rosters. LPA conducted a physical plant tour at approximately 11:30 AM and the following was noted:

There are two entrances being utilized at the facility. One entrance is utilized by both staff and participants. The second entrance is utilized by visitors. Both entrances are located within the front of the facility. The facility is a single unit building with Administrative/staff offices, activity rooms, bathrooms and kitchen. There are ten (10) activity rooms, total of eight (8) bathrooms, five (5) staff offices, a janitor's closet, four (4) storage rooms, a kitchen/dining area, a kitchenette, two (2) conference rooms and a wellness room.

The facilities’ capacity is one hundred (100) clients, and during the inspection, sixty-two (62) clients were present. The licensed facility has a fire clearance for seventy-five (75) ambulatory clients and twenty-five (25) non-ambulatory clients. The facility is vendor through North Los Angeles Regional Center (NLARC).

Common areas: The common areas include but are not limited to: the activity area, media area, resting area, arts/craft area, loungeroom, bathrooms, kitchen and Administrative/staff offices. The common areas were observed to be neat, clean and organized. LPA observed all rooms to be properly furnished and in good repair. LPA observed several tables and chairs for clients' use. The facility maintains a comfortable temperature at 72°F. (continued on LIC 809-C)
