The inspector’s account

At 09:30 AM, Licensing Program Analyst (LPA) Huma Rahimi conducted an unannounced annual inspection. LPA met with the staff Moises Quintana, and the Administrator arrived later. LPA disclosed the reason for the visit. LPA toured the facility with the staff and observed the following:

The facility has four (4) bedrooms and two (2) bathrooms, there is no live in staff. The facility is Fire Cleared for four (4) ambulatory and has awake staff.

Kitchen: LPA conducted a tour of the kitchen and observed there to be sufficient stock of two-day perishable and seven-day non-perishable foods. Frozen foods are properly wrapped and stored. Food storage and preparation areas are clean and inaccessible to pests. LPA observed all cleaning supplies and knives are locked under the sink in the kitchen and inaccessible to clients. The medications are stored in a locked cabinet in the kitchen. The Fire extinguisher was observed to be full and last serviced on 06/25/2025.

Common Areas: Walls, floors, ceilings, windows, screens, and blinds were clean and in good repair. At 9:55 AM LPA measured the room temperature to be 75 degrees. LPA observed postings in the dining room for the facility’s house rules, grievance procedures, Emergency Disaster Plan, facility sketch, and personal rights.

Bedrooms: The facility has four (4) bedrooms. All bedrooms are private. All bedrooms contained a nightstand, lamp, storage, and bed with adequate bedding. All furnishings were clean and in good condition.
