Facility Evaluation Report
On 6/17/2026 at approximately 11:10 AM, Licensing Program Analyst (LPA), Angelica Segovia conducted a Case Management visit during their subsequent complaint visit (Control# 31-AS-20260306141233) and the following deficiency was observed.
Upon LPA's arrival the Administrator, Yuson Jhomer was not available to assist with today’s visit. LPA spoke with the Licensee, Anderson Munoz who designated one of the care staff to sign today’s report. LPA provided the findings of the Case Management visit to the Licensee.
Regarding LPA’s supplementary record review of photographic images sent to Community Care Licensing Division, (CCLD), said images showcased bruising on C1’s arms and chest. LPA’s interview with S1 on 4/21/2026 revealed they were made aware of C1’s chest bruise but stated C1 could have possibly “self-injured” themselves due to becoming “super aggressive” while staff was conducting a CPI. When questioned if they had submitted an Unusual Incident Report (SIR) regarding C1’s bruising, S1 stated, “No, it is my bad”.

No other immediate health and safety issues observed during the day of the visit. Exit interview was conducted, appeal rights given, and a copy of this report was provided to the Administrator.
