Facility Evaluation Report
On 2/02/2026 at approximately 02:00 PM, Licensing Program Analyst (LPA), Angelica Segovia conducted an annual visit to the facility. LPA was greeted by staff member, Godson Alinde. The Administrator, Margaret Kayiira was unavailable and designated the staff member to assist with today’s visit.
LPA asked for the census. LPA conducted a physical plant tour at approximately 02:30 PM and the following was noted:
The facility is a single unit building with four (4) bedrooms and two (2) bathrooms currently occupying zero (0) clients. There is no designated staff room. The facility has an approved fire clearance for four (4) ambulatory clients. The facility is awaiting vendorization through North Los Angeles Regional Center (NLARC) to operate as an Adult Residential Home.
Common areas: The living room and dining room were observed to be properly furnished and in good repair. The facility maintains a comfortable temperature at 68°F. LPA observed a fire extinguisher to be located near the kitchen. LPA observed required postings located throughout the common areas. A working telephone was observed. LPA observed the fireplace to be covered and inaccessible to clients.
Kitchen: The kitchen was observed to be kept in proper condition. The Administrator will ensure sufficient supplies of seven (7) day nonperishable foods and two (2) day perishable foods will be readily available upon the arrivals of clients. The knives/sharps and medications will be kept within a locked kitchen cabinet. (continued on LIC 809-C)
