Facility Evaluation Report
On 06/15/2026, Licensing Program Analysts (LPAs) Lorena Casillas and Evelin Rios conducted an unannounced case management visit. LPA met with the Administrator Belinda Rene Hicks and disclosed the reason for the visit.
Today’s case management visit is to follow on an unusual incident previously reported on 06/02/26 by the facility in which Client #1 (C1)’s hair was allegedly pulled by Staff #1 (S1). The purpose of the visit is to obtain additional information regarding this incident. Between 10:00am to 12:00 pm LPA interviewed Administrator and two (2) staff.
It was reported that on 06/01/26 C1 had a behavior episode where C1 kept banging their head during a behavior episode. Due to this behavior S1 allegedly pulled C1’s hair, however based on Special Incident Report (SIR) it was reported that an internal investigation was conducted and the alleged incident did not take place as originally assumed. LPAs interviewed Administrator who stated that C1 was in fact having a behavior where C1 kept banging their head against the wall, this is documented behavior for C1. However, Administrator stated that S1 denied pulling C1’s hair, instead S1 explained that in order to prevent C1 from injuring their head, S1 attempted to place their hand as a barrier between C1’s head and the wall, to minimize injury. As S1 was reaching for C1’s head, S1’s hand was tangled in C1’s hair. The Administrator further stated that it was S1’s immediate reaction to prevent C1 from causing self injury. Normally staff grab a pillow to minimize injury, but the incident happened so fast that S1 simply reacted as best they could to make sure that C1’s head was not injured.
