Facility Evaluation Report

On 03/19/2025 at 10:50 a.m., Licensing Program Analyst (LPA) Evelin Rios conducted an unannounced annual visit and was greeted by the Administrator, Andrea Wallace. LPA stated the purpose of the visit. The Administrator stated there are no clients living at the facility. LPA Rios toured the facility and did not observe clients in the facility.

LPA accompanied by Administrator began the physical plant tour of the facility inside and out at 10:56 a.m. LPA toured four (4) client bedrooms and observed all client bedrooms were furnished with a bed, linens, lamp, night stand, and chair. LPA observed the common areas such as the dining room and living rooms still contained seating for the capacity of the facility. LPA observed a filing cabinet that is able to lock will store client records. LPA observed a fully charged fire extinguisher with a last serviced date of 09/11/2024. LPA observed the administrator test four (4) smoke detectors and LPA tested two (2) carbon monoxide detectors through out the facility at 11:05 a.m. LPA observed the detectors were operable. The staff room/office contained furniture and will store facility files. The facility has fully stocked first aid kits. The facility has three (3) bathrooms two (2) are accessible to clients and were observed to have toilet paper, hand soap, and paper towels. LPA measured the hot water temperature at 11:23 a.m. in one bathroom and it read 109.6 degrees Fahrenheit, within regulation. LPA's tour of the backyard and side yard, observed there is shaded area for clients with patio furniture for clients use. Passageways were clear of clutter or hazards.

LPA reviewed the administrator certificate on the CCLD website. Review revealed administrator certification is active.

There were no deficiencies observed at this time. Exit interview was conducted and a copy of the report was given to the Administrator.
