Facility Evaluation Report
Licensing Program Analyst (LPA) Lorena Casillas met with Administrator George McKay for an unannounced one (1) year Required visit for this facility.
LPA arrived at 09:40 am. The administrator was not in the facility or the area nor was there staff to grant access. LPA called Administrator and Administrator agreed to meet later. Administrator arrived shortly after and allowed entry. Entrance interview conducted. The tool kit was not used for this visit since there are no clients.
A tour of the physical plant was conducted with the Administrator at 12:40 p.m. The facility has four (4) bedrooms and two (2) bathrooms. It is currently not occupied by clients. Facility is fire cleared for four (4) ambulatory clients. Facility will have awake staff.
Living and Dining: LPA observed the living room furniture to be clean and in good repair. LPA observed the dining area to be clean and in good repair. The facility maintains a comfortable temperature at 70 degrees F. The smoke alarm and carbon monoxide detector were operational and tested at 12:50 pm. The fire extinguisher is full and last serviced on 08/29/24. Files were observed to be in a locked cabinet located in the dining room.
Kitchen: LPA conducted a tour of the kitchen at 01:00 pm and observed there to be sufficient stock of two-day perishable and seven-day non-perishable foods. Frozen foods are properly wrapped and stored appropriately. Food storage and preparation areas care clean and inaccessible to pests. Medications, knives, and sharp objects are kept centrally stored and locked in a cabinet located in the kitchen and in a cabinet towards the rear of the house.
