Facility Evaluation Report
Licensing Program Analysts (LPAs) Mayra Cota and Blanca Gonzalez, conducted an unannounced annual visit at the facility met with Andrea Barrera and explained the reason for the visit.
The facility is an Adult Day Program (ADP) licensed to serve fifteen (15) ambulatory and six (6) non-ambulatory adults ages 18 years and above. The program is located in a business area of Lakewood and consists of 1 large single-story building.
The facility consists of the following: reception area, conference room, 2 administrator offices, break room, leisure room, arts and crafts room, kitchen, computer room, quiet/relaxation room, 2 storage rooms, copy room, style room, 2 restrooms, and a locker area.
LPA stoured the facility with Leonel Vazquez and the following was observed:
A total of 17 consumers were present at the facility during the visit. The staff to client ratio is 1:3 and ratios were observed during visit. The facility is clean, safe, sanitary and in good repair. All passageways are free from obstruction.
Consumer activity rooms were observed to be clean and there is sufficient seating for activity engagement. Bathrooms were clean and stocked with personal hygiene products. Water temperature was tested in bathrooms, and measured within the required 105-120 degrees F. Disinfectants, cleaning solutions and toxins are inaccessible to clients and are locked inside a storage room.
