Facility Evaluation Report
On 7/13/26, Licensing Program Analyst (LPA) Jewel Baptiste conducted an unannounced Annual/Required inspection to Santos Home- Lakewood. Upon arrival LPA was greeted by DSP Jay Samilin, and LPA explained the reason for the visit. The administrator Ma Rhona Amado arrived (1) hour later and assisted with the visit. There were (3) clients in care during the time of this visit, but they all left to the day program.
The last emergency disaster/fire drill was conducted on 5/10/26. The Administrator Certificate expires on 03/16/2028. The facility is licensed to serve 4 clients between the age of 18 and 59 years old, ambulatory only. The facility is vendorized through Harbor Regional Center.
During today's visit LPA inspected the physical plant inside and outside, reviewed the food supply, tested the smoke/carbon monoxide detectors, reviewed (3) staff files, (3) client files, medications, medication administration records for (3) clients and P&I for (3) clients. LPA also reviewed infection control plan, and emergency disaster plan.
The facility is a single-story building in a residential area with a kitchen, dining room, Living room, 2 client bedrooms, 1 staff bedroom, 2 bathrooms, laundry space, attached garage, and a backyard. LPA toured the physical plant with the DSP, and observed all (2) client bedrooms, containing required furniture, lamps, dresser, chair, and closet space. The two bathrooms contain a working toilet, basin, and water faucet, walk in shower with grab bar. The temperature measured at 106.3*F. The smoke detectors were battery operated and tested and observed to be working properly.
