The inspector’s account

Licensing Program Analyst (LPA) Christian Gutierrez conducted the annual inspection using the Compliance and Regulatory Enforcement (CARE) tools. LPA met with Rhona Amado (Assistant Administrator) and explained reason for visit. Administrator Jay Samilin was notified by telephone.

The facility is licensed to serve 3 ambulatory and 1 non-ambulatory developmentally disabled clients ages 18-59 years old, restricted health conditions. Clients receive services through the Harbor Regional Center. The facility is in a residential area of Lakewood. A tour of the single-story facility included the living room, dining area, kitchen, 4 client bedrooms, 1 staff room, 1 storage room, 2 bathrooms, attached garage, front yard, and backyard.

LPA toured the facility and observed the following: Each client bedroom has the required bedding and furniture. There is extra clean linen in hallway closet. LPA observed two small holes above bed in bedroom #2 Smoke detectors and carbon monoxide detectors were observed in each room and throughout the facility. LPA observed Bedroom #1 smoke detector not working. The facility has one (1) fully charged fire extinguishers which is kept in kitchen. Cleaning supplies and toxic substances are inaccessible to clients in a locked cabinet in kitchen as well as sharps and knives. Freezers are maintained at a temperature of 0-degree F and the refrigerators at a maximum of 45 degrees F. Sufficient supply 7 days non-perishable foods was observed in the kitchen. LPA did not observe a sufficient supply of fresh food for two days for four clients. There are no firearms or weapons stored at the facility. The hot water temperature in the bathrooms were measured not between the required range of 105-120 degrees F. Bathroom #1 had a reading of 137.1 and Bathroom #2 138.2. The facility does not have a swimming pool or bodies of water on the premises There is a shaded seating area for the residents located in the backyard. Passageways and exits are free of obstruction.
