Facility Evaluation Report
Licensing Program Analyst (LPA) De Christian Gutierrez conducted a case management visit at the facility to check on the health and safety of the Residents in care. LPA met with the Caregiver Julie Hunter who assisted with the visit. Administrator Lilian Okereke was contacted via text and stated she was currently in a meeting.
During the visit, LPA conducted a health and safety check of the six (6) residents which include tour the facility. LPA did not observe any health and safety concerns or issues with residents in care based on Title 22 regulation.
No deficiencies were observed during today's visit. Exit interview was held and a copy of the report was provided.
