Facility Evaluation Report
Licensing Program Analyst (LPA) Christian Gutierrez conducted the annual inspection using the Compliance and Regulatory Enforcement (CARE) tools. LPA met with Administrator Jorgelle Catunao and explained reason for visit.
The facility is licensed to serve four (4) non-ambulatory developmentally disabled adults age 18 to 59, with restricted health conditions Clients are serviced by Harbor Regional Center. The facility is in a residential area, A tour of the single-story facility includes 4 client bedrooms, 1 staff room, 2 bathrooms, living room, dining area, kitchen, front yard, backyard and detached garage.
LPA toured the facility and observed the following: Each client bedroom has the required bedding and furniture. LPA observed C1’s bedroom to have dirty sheets, trash on floor, and soiled items wrapped up on floor. There is extra clean linen and towels in hallway closet. Smoke detectors and carbon monoxide detectors were observed in each room and throughout the facility and are properly operating. The facility has one (1) fully charged fire extinguishers located in kitchen. Cleaning supplies, toxic substances, and sharps are inaccessible to clients in locked cabinets in kitchen. LPA observed an unlocked closet with medication accessible to clients. Freezers are maintained at a temperature of 0-degree F and the refrigerators at a maximum of 45 degrees F. LPA observed a sufficient supply of 2-day perishable & 7 days non-perishable food. There are no firearms or weapons stored at the facility. The hot water temperature in the bathrooms did not measure between the required range of 105-120 degrees F. Two staff did not have required health screenings. LPA checked client’s P/I funds and discovered money not accurate. The facility does not have a swimming pool or bodies of water on the premises There is a shaded seating area for the residents located in the backyard. Passageways and exits are free of obstruction.
***Due to time constraints, LPA was not able to complete the annual inspection for this facility. LPA will do a continuation of this inspection. ***
Deficiencies have been noted on LIC 809D under Title 22 Regulations. Exit interview was conducted and a copy of this report, LIC 809D and appeal rights were provided to Administrator.
