Facility Evaluation Report
Licensing Program Analyst (LPA) Sandra Urena arrived at the facility unannounced to conduct a required annual inspection. At the time of arrival there was no answer and the LPA contacted the Administrator Pauline Mortel via telephone and explained the reason for the visit. The Administrator stated that staff were out in the community at a doctor’s appointment, and that staff would be back in an hour, and the Administrator was not available to come to the facility. The LPA stated they would come back in an hour to proceed with the annual inspection. This is a level II facility vendored through the North Los Angeles County Regional Center.
The LPA and the staff toured the physical plant areas inside and outside to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations.
COMMON AREAS: At the time of the visit, living room and dining room furniture was observed to be in good condition. The facility maintained a comfortable temperature. Smoke detector(s) and carbon monoxide detector were tested and operational at the time of the visit. The fire extinguisher(s) were fully charged and were last purchased on 04/16/2025. The LPA observed required postings throughout the common space.
KITCHEN: The kitchen appeared to be clean at this time and the appliances and fixtures functional during the time of visit. LPA observed a sufficient amount of perishable and non-perishable food at the facility; properly stored. Sharp objects are stored in a locked cabinet above the washer and dryer in the laundry area right next to the kitchen. No Cleaning supplies and toxins were observed in this area at this time. To the right of the kitchen was the office area and dining room. Dining furniture in kitchen and dining room area appeared to be clean and sufficient at this time.
