The inspector’s account

It is alleged that a staff (S1) hit a resident (R1) on the legs with a broom, however no injuries were sustained. To investigate the allegation, LPA Urena conducted record reviews and interviews. On 07/09/2025, LPA Urena interviewed staff from the North County Los Angeles Regional Center (NCLARC) from approximately 4:09 p.m. to 4:56 p.m. The interview revealed that NCLARC staff received information from the hospital that no injuries or bruises were observed during the admission medical examination of R1 on 06/26/2025. The facility is vendored through NCLARC. NCLARC conducts independent investigations, and the investigation conducted on 06/27/2025, by the Quality Assurance (QA) staff revealed that staff denied any inappropriate behavior towards R1. Furthermore, interviews conducted by the QA revealed that two residents were present during the time of the incident, the residents reported that they are happy living at the facility and have positive relationships with staff, and did not observe any staff hit R1. The LPA interviewed the Administrator, and the interview revealed that R1 has been staying at the facility on a 21-day respite admission, and R1 has been in and out of the hospital due to R1 calling 911 for various reasons, but specifically for threats of self-harm. The LPA interviewed staff, and staff denied abusing R1 physically or emotionally.

Although the allegation may have happened or is valid, based on the interviews and observation, there is not sufficient evidence to prove the alleged violation(s) did or did not occur. Therefore, the allegation that the staff hit R1 with a broom causing injury and or bruising is deemed Unsubstantiated at this time.
