Facility Evaluation Report
Licensing Program Analyst (LPA) Sandra Urena arrived unannounced at the facility to conduct a Case Management-Other visit. The LPA met with Licensee Anna Hakobyan and informed them of the reason for the visit. The reason for today’s visit is to gather additional information on the Licensee’s intent to adapt the facility to the North Los Angeles County Regional Center (NLACRC) recommendations.
At the time of the visit, the Licensee and their family are residing at this location temporarily. Once NLACRC finalizes the contract and is ready to assign clients, Licensee and family will move out. There are no clients residing in the facility at the moment. The facility was licensed on 08/29/2024. The licensee stated that they applied to be vendorized through NLACRC.
During today’s visit, the Licensee stated that they are willing to make adaptations per recommendations by the NLACRC once the NLACRC confirms that they will approve the contract if the adaptations are made. The licensee stated that they will raise the ground to be at level with the bedrooms # 1, #2 and kitchen to ensure that the residents have easy access to the side of the facility and accommodate residents with mobility challenges. Licensee stated that they would open up a door to lead to the side of the facility and create an additional outdoor area for the facility. The area would be fenced to prevent access to the in-ground pool. The distance between the front door of the Adult Residential Facility (ARF) and the back of the Residential Facility for the Elderly (RCFE) is 14 feet. Front door of the ARF has a ramp installed. The Licensee stated that they would create an additional egress gate at the front of the facility,and would add a fence to separate the ARF egress gate from the RCFE gate.
Exit interview was conducted and a copy of the report was issued.
