The inspector’s account

Licensing Program Analyst (LPA) Sandra Urena arrived unannounced at the facility to conduct the required annual inspection. The LPA was greeted by the Administrator Justin Levi and informed them of the reason for the visit.

The facility was licensed on 09/10/2024. The Administrator stated that they applied to be vendorized through the North Los Angeles County Regional Center (NLACRC) and received a final visit from the Quality Assurance (QA) team. NLACRC team stated that facility application may be approved by the start of September 2025. This facility will be housing clients from Regional Center, and it will be classified as a Level 6 housing. Regional Center application is in process and not approved at the time of the visit. No residents are currently residing at the facility.

The LPA and the Licensee toured the physical plant areas inside and outside to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations.

COMMON AREAS: At the time of the visit, living room and dining room furniture were observed to be in good condition. The facility maintained a comfortable temperature. The fire extinguisher was fully charged and was last serviced on 01/27/2025. The LPA observed required postings throughout the common space. Additionally, a detached laundry room was observed. Phone and internet system will be connected before the first client is admitted
