Facility Evaluation Report
LPA Gabriela Castro conducted an unannounced Case Management visit at the facility. LPA was greeted by facility staff, and Laura Ignacio, Administrator/Licensee was contacted via telephone.
The purpose of the visit was to confirm information received by the Department regarding the facility's anticipated closure. During the visit, LPA discussed the 30-day eviction notices that had been submitted to the Department and informed the Licensee that the notices did not meet regulatory requirements. LPA provided technical assistance regarding the applicable regulations and advised the Licensee that the eviction notices must be reissued with the necessary corrections and updated dates to ensure compliance. LPA answered the Licensee's questions regarding the facility closure process and the requirements for providing proper notice to residents. LPA advised the Licensee to follow the appropriate eviction procedures and informed the Licensee that the Department will continue to monitor the facility's compliance. LPA will maintain communication with the Licensee and follow up to ensure the corrected eviction notices are issued and that the facility closure proceeds in accordance with applicable licensing regulations.
