Facility Evaluation Report

Licensing Program Analyst (LPA) Cynthia Chan conducted a case management visit to follow up on the incidents reported on 1/7/26. LPA arrived unannounced and met with the administrator, Melissa Villanueva. The purpose of the visit was explained.

The facility submitted incident reports to licensing on 1/7/26 to inform that an allegation was made against staff members locking client #1 in the bedroom. In addition, it noted that staff put hot sauce in clients' food and was given little water when clients have behaviors.

During the visit today, LPA toured the facility, reviewed food supplies, and client files. LPA observed clients' bedrooms have door handles without locks from either side. Food supplies are sufficient.

LPA interviewed the administrator and three (3) staff today. Staff interviewed have not observed anybody locking the clients in their rooms or preventing them from exiting their rooms. Staff stated that the clients go in and out of their rooms throughout the day. The clients were not available for interviews. LPA will interview the clients another day.

An exit interview was held and a copy of this report was given to the administrator.
