Facility Evaluation Report

Licensing Program Analyst (LPA) Cynthia Chan conducted a case management visit to follow up on the initial case management visit dated 6/17/25 regarding the death of a client. LPA met with Alma Enriquez, the administrator, and explained the reason for the visit.

The initial visit was conducted on 6/17/25 to obtain documentation for Client #1 (C1), who passed away at the facility on 6/13/25. LPA interviewed the administrator and a staff. The information gathered revealed that C1 had developed a low-grade fever a week prior and was experiencing difficulty breathing. On 6/11/25, C1 was placed on hospice with a 24-hour hospice nurse to monitor the client. C1 passed away on 6/13/25 at the facility.

According to the death certificate, C1’s cause of death was listed as cardiopulmonary arrest, hypertension, seizure disorder, and Alzheimer’s disease.

Based on information gathered, there was no evidence indicating that the facility contributed to the client’s death.

An exit interview was held and a copy of this report was given to the administrator.
