Facility Evaluation Report

Licensing Program Analysts (LPAs) Cynthia Chan, Gabriela Castro, and Elena Mallett conducted a case management visit due to the death of a client. LPAs arrived unannounced and met with Staff, Anabel Pastor. The reason for the visit was explained.

The facility reported the death of Client #1 (C1) on 6/13/25. C1 received documentation such as the Face Sheet, Physician's Report, IPP, and medication records from March through June 2025. Interviews were held with the administrator and Staff #1.

C1 passed away on 6/13/25, 6:04 a.m. at the facility. Per staff, C1 developed a low grade fever a week prior and had difficulty breathing. C1 was placed on hospice on 6/11/25 and was receiving comfort care.

LPAs toured the facility and did not have any health and safety concerns. LPA requested for the death certificate to be emailed when obtained.

An exit interview was held and a copy of this report was given to the administrator.
