Facility Evaluation Report
Licensing Program Analysts (LPA) Nune Margaryan conducted an unannounced annual visit at the facility using the CARE tool. LPA met with Administrator Raul Diaz and explained the reason for the visit. The facility is licensed as an Adult Day Program to serve 150 developmentally disabled adults clients ages 18 and older. Clients in the day program receive services provided by San Gabriel / Pomona Regional Center and East Los Angeles Regional Center. The day program is a large campus, located in a residential neighborhood. The facility consists of a Main Activity Building, Warehouse #2, Outdoor Storage Shed, Administrative Building, Accounting Building. Main Activity Building consists of few areas for different activities, library, theater, front office, staff offices, staff break room, dining room, kitchen, 5 clients bathrooms, custodial closet, warehouse #1. LPA toured the facility with Administrator, and the following were observed: The program site is clean, safe, sanitary and in good repair. All passageways are free from obstruction. Disinfectants and cleaning solutions are inaccessible to clients and are locked in the custodial closet. Sharps are locked in the kitchen and inaccessible to clients. Multiply fire extinguishers are located throughout the facility and are fully charged. The carbon monoxide and smoke detector was observed to be operational. The bathrooms were observed to be clean and operational. The water temperature was tested in the bathrooms, which measured between 105 - 120 degrees F. Food is not prepared at this program and clients are able to bring their own food and staff would assist them to heat up the food during meal time. Refrigerators were observed at the facility, to be operational and used for consumer snacks, drinks, and any food they may bring that needs to be stored. There are storage cabinets/ lockers for the clients and staff belongings.
