Facility Evaluation Report

Licensing Program Analyst (LPA) Nune Margaryan conducted an unannounced case management visit to this facility. The purpose of the visit was to follow up on an incident report received by Community Care Licensing (CCL) on 03/19/25. LPA met with Administrator Raul Diaz and explained the reason for the visit.

According to the report on 03/18/25 C2 inappropriately touched C1 over the clothing during day program. No staff or clients witnessed the incident. Administrator spoke with both clients and their Representative Parties and SIR was submitted to CCL and RC. Police was notified. Police Officer took the statements from the staff and report number was provided. At the time of the visit LPA met with C1's Family Member (FM) who came to pick up C1. LPA spoke with FM, and they stated that Facility Administrator follow up on the incident, spoke with them and by FM request C1 and C2 were reassigned to different groups to avoid them having breaks and lunch together.

During today’s visit LPA toured the facility, Interviewed Administrator, C1 and C2, reviewed C1's and C2's files and obtained copies of relevant documents pertaining C1 and C2. Interviewed C1 And C2 stated that they enjoying being at the day program. C1 stated that they feel safe.

LPA toured the facility and there were no health and safety issues observed at the facility.
