Complaint Investigation Report
Allegation: Staff physically abused clients in care. It is alleged that staff are neglecting and abusing clients in care. LPA Interviewed three (3) clients and two (2) of three (3) were not able to corroborate the allegation. One (1) client refused to be interviewed. One (1) client stated the client was pushed into bed by one staff for acting up. Review of the behavioral data shows two (2) clients have a history of fabricating allegations, and there were no witnesses to corroborate the allegation. LPA interviewed ten (10) staff members, and all ten (10) staff members denied the allegations. Body checks are done daily to document any injuries to clients. The Administrator stated she always makes random visits at all hours of the day to make sure clients are safe and staff are doing their jobs. There is insufficient evidence to substantiate this allegation.
Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated.
Exit interview was conducted and a copy of the report was provided.
