The inspector’s account

Licensing Program Analyst (LPA) Cynthia Chan conducted a case management visit regarding the renovation of a bathroom in September of 2025. LPA met with Administrator Angela Williams and explained the reason for the visit.

The facility reported the renovation of one of the bathrooms at the facility back in September 2025. The administrator had previously submitted the initial project plan and permit for the construction. The project was completed in October 2025.

During the visit today, LPA conducted a walk through and observed the bathroom fully remodeled. The administrator provided the inspection record, and the final inspection was conducted on 10/20/25 and 10/31/25.

An exit interview was held and a copy of this report was given to the Administrator.
