Facility Evaluation Report
An informal conference was held at the Monterey Park Adult and Senior Care Program Regional Office on 2/18/26. The individuals present for the meeting were: Fernando Fierros (Licensing Program Manager), Cynthia Chan (Licensing Program Analyst), Lee Strollo (Facility's District Manager), Serena Rojas (Vice President of Redwood Family Care Network), Marissa Marti (Regional Director), and Angela Williams (Facility Administrator).
The purpose of the meeting is to discuss the citation issued for staff causing injury to the client in care during a Crisis Prevention Intervention (CPI) performed on 07/01/2022. The department concluded the investigation on 10/05/2023, and a deficiency was issued for 85102(a)(5) Emergency Intervention Prohibitions, along with an immediate $500 civil penalty. The community care licensing department has determined that an additional civil penalty will be assessed due to causing injury to a client in care. The additional civil penalty will be delivered at a later date and time.
The licensee representative has been advised that the department is seeking an additional civil penalty. An exit interview, and a copy of this report was provided to Lee Strollo.
