The inspector’s account

Regarding the allegation: Staff hit resident. It is alleged that staff S#2 hit client C#1. Interviews conducted by LPA Villalobos, nine (9) out of (9) staff deny this allegation and two (2) out of (2) clients could not corroborate the allegation. Clients at the facility have intellectual disabilities and did not recall the events that allegedly took place. According to the IPP for C1, consistent behaviors patterns of fabricating events(fibbing to gain attention). Staff state they handle the clients with respect and dignity, providing them with assisted daily living to become more productive and responsible. Based on interviews conducted by LPA Villalobos. Physicians report, IPP documentation, and staff interview(s) reviewed, and Regional Center Service Coordinator interviewed by LPA Vaid. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is unsubstantiated.

Regarding the allegation: Staff did not take the resident to get medical care. It is alleged that the facility staff did not seek emergency care for the client #1(C1) after S2 had allegedly hit C1. Nine (9) out of (9) staff interviewed by LPA Villalobos deny this allegation and two (2) out of (2) clients could not corroborate this allegation. According to the Administrator, on 03/05/24 and 03/07/24 paramedics and sheriffs were called out to the facility for alleged attack on C1 by S2. Both incidents were reported to licensing, and during both times paramedics conducting body check did not find bruising nor redness on C1 body from the alleged attack. On 03/07/24 after C1 mentioned being attacked by staff, C1 was taken to hospital for check-up for internal injuries. C1 returned the same day. Based on interviews conducted by LPA Villalobos. Physicians report, IPP documentation, and emergency department patient visit information packet reviewed by LPA Vaid. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is unsubstantiated.

An exit interview was conducted and a copy of this report was given to Angela Williams, Administrator.
