Facility Evaluation Report

Licensing Program Analyst (LPA) Cynthia Chan conducted a follow-up case management visit regarding a reported incident. LPA met with Staff and explained the purpose of the visit.

LPA Chan conducted the initial case management visit on 9/4/25 regarding this self-reported incident. The facility reported that allegedly Staff #1 (S1) told Client #1 (C1) that the staff would withhold dinner if C1 is not good. During the initial visit, LPA interviewed S1 who denied saying those words to C1. The other 3 staff interviewed have not heard S1 saying those words either.

During today's visit, LPA interviewed additional 2 staff and 4 clients. Staff and clients interviewed have not heard any staff tell the client they will withhold their food or make any threats toward the clients.

Based on the information gathered, there is no supporting evidence of staff threatening to withhold the client's dinner. Therefore, the allegation is unsubstantiated.

No deficiency is issued today. An exit interview was held and a copy of this report was given to the Staff Rios.
