Licensing Program Analyst (LPA) Cynthia Chan conducted a case management visit for an incident that was reported on 8/28/25. LPA arrived unannounced and met with Administrator Janet Trujillo.
The self-reported incident indicated that Staff #1 (S1) told Client #1 (C1) that the staff would withhold dinner if C1 is not good. LPA interviewed S1 who denied saying those words to C1. The other 3 staff interviewed have not heard S1 saying those words either.
Clients were not available for interview today. Therefore, additional interviews are needed.
An exit interview was held and a copy of this report was given to the administrator.
