Facility Evaluation Report

Licensing Program Analyst (LPA) Cynthia Chan conducted a case management visit regarding a reported incident that occurred at the facility. LPA met with Administrator, Janet Trujillo, and explained the purpose of the visit.

The regional office received an incident report from the facility on 6/26/26 regarding an incident that involved a staff and a client. The report indicated that on 6/26/26, Client #1 (C1) became agitated with Staff #1 (S1). C1 became verbally aggressive with S1 and then struck the staff on the upper right arm. S1 immediately reacted by hitting C1 on the back with a closed hand.

Per the administrator, S1 was interviewed and admitted to striking C1 in the back as a reaction to the contact. Administrator also stated that two staff, who were on shift, witnessed the incident on that day. LPA received copies of the written statements by staff. S1 was terminated on 7/2/26.

LPA interviewed two (2) clients during the visit today. C1 admitted to hitting S1 first and stated that S1 hit back. The other client heard about the incident but stated that staff have never been physical towards the client.

Based on interviews and statements reviewed, S1 made inappropriate contact with C1. A deficiency is being issued today. An exit interview was held and a copy of this report was given to the administrator.
