Facility Evaluation Report
Licensing Program Analyst (LPA) Cynthia Chan conducted a case management visit for an incident that was reported on 8/28/25. LPA arrived unannounced and met with Administrator Janet Trujillo.
The self-reported incident indicated Staff #1 (S1) told Client #1 (C1) that the staff would give a “pow-pow”. LPA interviewed S1, who recalled a time when C1 was seeking attention and knocking down the books off the wall. S1 did not recall the date; however, admitted that C1 was told that the client would get a “pow-pow” if C1 knocks off the books again. S1 stated there was no mal intent and the “pow-pow” meant a slight tap on the hand.
LPA interviewed an additional 3 staff members, and they have not witnessed any staff being verbally aggressive or using inappropriate words with clients.
A deficiency was issued on the LIC809D due to staff speaking inappropriately to the client. An exit interview was held, and a copy of this report, along with appeal rights, was given.
