The inspector’s account

Licensing Program Analyst (LPA) Cynthia Chan conducted the unannounced annual inspection on 8/19/25. LPA met with Administrator, Marilyn Sabio, and informed the reason for the visit. The facility is licensed for (12) clients, ages 18 through 59, ambulatory only. The fire clearance approves six (6) clients in the #15027 building and six (6) clients in the #15031.

LPA utilized the Compliance and Regulatory Enforcement (CARE) tools to inspect the facility.

The facility consists of two separate houses: House #1 (front - #15031) consists of 3 client bedrooms, 1 staff bedroom, 2 bathrooms, main office, enclosed porch, living room, kitchen, and laundry area. House #2 (back- #15027) has 4 bedrooms, 1 staff room, 2 bathrooms, and a living room. No swimming pool on the premises. There are no obstructions to the walkways. The fireplace is not in usage and inaccessible to clients. Sufficient food supplies of two days of perishables and a week of non-perishables are observed. Knives and cleaning supplies are locked. The hot water temperature was measured within the range of 105-120 degrees F. Extra hygiene supplies are locked. Facility has an operable signal system. The fire extinguishers were last inspected on 7/31/25.

Medications are centrally stored and locked in the kitchen cabinet. LPA reviewed medications for 4 clients and staff are administering the medications as prescribed. There are no clients with a restricted health condition. LPA reviewed files for 4 clients. Client files have the required documents and the Needs/Services Plans are updated. LPA reviewed 2 staff files and files are complete. The administrator's (Marilyn Sabio) certificate expires on 4/28/26. The Emergency Disaster Plan is being reviewed annually and facility is conducting emergency drills at least quarterly.

No deficiencies were issued today. An exit interview was held and a copy of this report was given to the administrator.
