Facility Evaluation Report

Licensing Program Analyst (LPA) Cynthia Chan conducted an unannounced Case Management Visit. The purpose of the visit is to verify the facility closure, which was initiated by the licensee.

On 5/7/26, the licensee informed LPA of the intent to close the facility. There were five (5) clients residing at the home during that time. License provided verbal and written eviction notices to the clients. LPA verified that all five (5) clients were relocated to other assisted living facilities.

During the visit today, LPA toured the physical plant of both addresses, 15031 (front) and 15027 (back). LPA confirmed there are no clients residing at the facility and that their belongings are no longer there. Medications and P&I monies were distributed to the corresponding facilities when clients moved out. The back house is currently being leased to a private family.

The original license was collected at the time of the visit.

An exit interview was held. A copy of this report was given to Marilyn Sabio.
