Facility Evaluation Report
Licensing Program Analyst (LPA )Christine Wong conducted an annual required visit. LPA met with staff/DSP Obiamamaka Ojiakor (Obi) and explained the reason for the visit. Shortly after, the administrator Anthony Johnson arrived and assisted with the visit. LPA used the infection control tool to evaluate the facility. LPA observed the facility plant, COVID-19 procedures and observed food supply. Facility has submitted a mitigation plan and it was approved on 04/10/21.
The facility is a one story house and located in a residential neighborhood area. The house consists of living room, dining area, kitchen, four clients bedrooms, two bathrooms and a detached garage. The front and back yard are maintained well. The front yard has shaded area with table and chairs for clients to use. All 4 client bedrooms were toured. Each bedroom has one bed, one chair, one night stand, drawer and required furniture and linen and sufficient lighting and closet space. All 2 bathrooms were toured and they are clean, safe, sanitary and in a good operable condition. The hot water in both bathrooms were measured between 115.4 and 117.3 degrees F. The refrigerator and cabinet in the kitchen has sufficient two days perishable and seven days non-perishable food supply. All the sharp knives and utensils and cleaning supplies are kept in the garage and its inaccessible to clients. All the appliances in the kitchen are clean and working properly. The common areas such as living room and dining area are clean and have the required furniture. LPA also inspected the carbon monoxide detector which is mounted on the wall of the hallway and the smoke detectors are located in each client bedroom and common area. They are all working properly.
Facility is currently following COVID 19 recommendations regarding COVID 19 signs throughout the facility, facility is disinfected every time that used, the bathrooms have sufficient soap, paper towels, and signs, and PPE supplies are sufficient for more than 30 days.
An exit interview conducted. A copy of the report will be provided to administrator Anthony Johnson.
