Complaint Investigation Report
In regards to the allegation: Staff force clients to go to bed. Interviews with 7 of 7 staff indicated that clients are not forced to go to bed. Interview with 1 client indicate that they go to bed when they want to.
In regards to the allegation: Staff are writing false information on reports. Interviews with 7 of 7 staff indicate that they have never written false information on a report nor have they witnessed other staff write false information on a report.
In regards to the allegation: Staff speak inappropriately to clients. Interviews with 7 of 7 staff indicate they have never spoken to a client inappropriately nor have they witnessed other staff speak inappropriately to a client. Interview with 1 client indicate that staff never spoken to them inappropriately.
In regards to the allegation: Staff are not assisting clients with hygiene needs. Interviews with 7 of 7 staff indicated clients are being assisted with their hygiene needs. LPA observed clients to be clean and well kempt. Interview with 1 client indicate that they are provided clean clothing and assistances with bathing.
In regards to the allegation: Facility staff are over medicating client. Interviews with 7 of 7 staff indicate that clients are not being over medicated. Medication assistances is based on doctor's orders. Interview with 1 client indicated that they are not being over medicated.
Based on LPA's interviews and observation, the investigation revealed: Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is unsubstantiated.
