Facility Evaluation Report
Licensing Program Analyst (LPA) Cynthia Chan conducted the unannounced annual inspection using the Compliance and Regulatory Enforcement (CARE) tools. LPA met with Staff and explained the purpose of the visit. The administrator Ademilayo Ridley arrived shortly thereafter. The facility is licensed for four (4) ambulatory adults, ages 18 to 59. The home is vendorized by the San Gabriel/Pomona Regional Center.
The facility has 4 client bedrooms, 2 bathrooms, living room, dining area, kitchen, and a garage. Client bedrooms are equipped with the appropriate furniture and storage space. The spacious backyard has a shaded area with table and chairs. There are extra linens and hygiene supplies. Facility has operable smoke and carbon monoxide combo detectors. Knives and cleaning solutions are locked. The hot water temperature was measured within 105-120 degrees F. Sufficient food supplies of 2 day perishable and a week of non-perishable observed. There are no items obstructing the walkways. Staff are providing care and supervision to meet the clients' needs throughout the shifts. The overnight staff is awake. Staff are fingerprint cleared and associated to the facility. LPA reviewed (3) staff files and files are complete. Administrator's (Ademilayo Ridley) certificate expires on 9/11/26. Staff files have current CPR & First Aid certificates and receiving appropriate annual training. Staff have CPI training. LPA reviewed (4) client files and their medications. All files contain the required documents. Medications are being administered as prescribed. Facility has the Emergency and Disaster Plan and conducting disaster drills at least quarterly.
No deficiencies issued today. An exit interview was held. Copy of this report was given to the administrator.
