Facility Evaluation Report
Licensing Program Analyst (LPA) Cynthia Chan conducted the annual inspection using the Compliance and Regulatory Enforcement (CARE) Tools. LPA arrived unannounced and met with the Administrator, Tim Seal, and explained the purpose of the visit.
The adult day program is licensed for a capacity of (50) developmental disabled clients, ages 18 to 59, of which (6) may be non-ambulatory. The facility is a two story building. First floor contains (1) large activity room with two restrooms and the second floor is used by staff only and contains staff offices, kitchen, and a restroom. The outdoor has a shaded area with tables and chairs and a garden. Sufficient space to accommodate both indoor and outdoor activities was observed. There are no bodies of water on the premises. The exit doors and walkways are free of obstruction. The hot water temperature was measured within 105-120 degrees F for the restrooms. There are lockers for clients to use. Cleaning supplies are locked and inaccessible to clients. Facility is continuing to follow their infection control guidelines.
LPA reviewed (4) personnel records. Staff have criminal background clearance and have current CPR & First Aid training. LPA reviewed (4) client files and the required documents were observed. There are currently no clients with a restricted health condition. Facility does not store or assist with medications. First aid kit was observed. The updated Emergency and Disaster Plan is in place, and facility is conducting drills at least every 6 months.
No deficiencies issued today. An exit interview was held and a copy of the report was given to the Associate Director.
