Facility Evaluation Report

Licensing Program Analyst (LPA) Erik Zaragoza conducted an unannounced Case Management Visit. The purpose of the visit was to verify facility closure and status of resident relocations. LPA met with Administrator/Licensee and President of the Licensee Robert Talens and explained the purpose of the visit.

· LPA confirmed there was no residents in the facility during visit.

· LPA confirmed resident’s belongings were not in the facility during the visit.

· Licensee surrendered the original license.

LPA confirmed that the previous two (2) residents of the facility relocated to the facilities El Espejo Home #197802585 and Colima Home #197802586.

The request for closure will be processed Per Title 22 Regulations, there were no deficiencies observed during the visit.

An exit interview was conducted with Alberto Lobarbio at her second home. A copy of the report was issued.
