Facility Evaluation Report
Licensing Program Analyst (LPA) Mariana Agban attempted to conduct a Required One (01) Year inspection of the facility. LPA arrived at around 10:30 A.M. and observed that there were no cars on the entire facility's campus. There was overgrown grass and closed windows that indicated the facility was abandoned. LPA walked from structure to structure in search of anyone. There were no signs of activity. LPA attempted to contact the facility Administrator multiple times, but no answer. LPA left a voicemail for the Administrator. Community Care Licensing (CCL) received on 07/31/24 the facility's Closure plan. Based on observations and inspection, and available information from the licensing records, the facility is closed and non-operational. Therefore, the facility file will be closed as of 06/06/2025.
