Licensing Program Analyst (LPA) Pamela Bunker conducted an unannounced Case Management-Annual Continuation visit to complete the Annual Required - 1 Year, dated August 29, 2025. On August 29, 2025, the same day, LPA Bunker returned to the facility and met with Licensee/Administrator David Vaughn and explained the purpose of today's visit. LPA Bunker continued to review medications, staff files, and residents' records.
