Facility Evaluation Report
On January 28, 2026, at approximately 12:00 p.m., an office meeting was held to discuss an unusual incident that occurred at the facility. Present at the meeting were Stephanie Cifuentes, Licensing Program Manager (LPM), Pamela Bunker, Licensing Program Analyst (LPA), Collette Ramirez, Administrator, and Clinese Davis Licensee, Arthur Khachatourians, Attorney .
During the meeting, LPM Cifuentes reviewed the details of the incident. On September 13, 2023, the Department issued deficiencies for a client receiving fractures in care.
At the time the findings were delivered on December 29, 2022, the Department indicated that an enhanced civil penalty determination was pending, pursuant to Health and Safety Code Section 1549(e)(1)(A).
Facility submitted an appeal for the two issued citations, one of which was dismissed and the other was upheld.
Licensee Clinese Davis remarked that she would have appreciated more information before the meeting as she felt blindsided by not being informed of the exact incident.
Licensee was informed the Department is reviewing the complaint for an enhanced civil penalty for physical abuse pursuant to H&S 1549(e)(1)(A). The total amount for the civil penalty is $10,000 for a serious bodily injury.
An exit interview was conducted, and a copy of this report was provided together with the appeal rights.
