The inspector’s account

On 4/4/2025, at 8:50 AM, Licensing Program Analyst (LPA) Bernadette Allen conducted an unannounced visit to conduct the required annual inspection. LPA was met by Sandra Moraga Administrator who was informed of the purpose of today’s visit and allowed LPA Allen into the facility.

The facility is an Adult Residential home licensed for the ages of 18 to 59, and approved for 2 non-ambulatory and 1 ambulatory resident. There are currently three (3) residents in placement.

Sandra Moraga Administrator was informed of facility fee’s that are due 4/16/2025.

The facility is a single-story home located in a residential neighborhood. The facility consists of three (3) bedrooms, two (2) bathrooms, living room, dining room,family room, two (2) backyard patio areas, laundry area and attached garage/office area.

At 9:15 AM, LPA Allen reviewed three (3) residents files for admission agreements, updated physician reports, and needs and services plans which were up to date. LPA Allen also observed the residents Medication Administration Records (MARS) which revealed that the residents are given their medications as prescribed by their physicians. The residents P&I was audited and funds on hand matched ledger.

LPA Allen reviewed four (4) staff files for First Aid/CPR certification, criminal record clearance, training's, and health screenings which were all current.
