Facility Evaluation Report
On 03/19/2026, Licensing Program Analyst (LPA) Bernadette Allen conducted an unannounced visit to conduct the annual inspection visit. LPA met with Cecelia Taylor- Direct Support Staff and she was informed of the purpose of today’s visit. Ava Brown arrived approximately at about 11:27AM.
The facility is a single-story home located in a residential neighborhood. The property consists of the following: 5 bedrooms, 4 bathrooms, living room, kitchen, dining room, den, washer and dryer and an outdoor shaded area in the front.
The facility is licensed to care for (5) clients in the age range of 18 through 59 of which (4) may be non-ambulatory in bedrooms #2, #3 #4, and #5, Bedroom #1 is for ambulatory only.
Currently, the home has (5) clients. The clients are Westside Regional Center clients. None of the clients have Restricted Health Care Conditions, and (1) is non-ambulatory. The facility does not handle any of the clients P&I.
At 11:30 PM, LPA reviewed five (5) clients files for admission agreements,IPP's, updated physician reports, and needs and services plan which appeared to be up to date.
LPA also reviewed three (3) staff files for First Aid/CPR certification, criminal record clearance, training, and health screenings which appeared to be up to date.
