Facility Evaluation Report

At 9:00 AM, Licensing Program Analyst (LPA) Bernadette Allen conducted an unannounced visit to conduct an annual inspection. LPA Allen was greeted by Damien Hodge- Support Staff upon arrival. LPA Allen introduced herself and explained the purpose of the visit and was allowed entry into the facility. At 9:40 AM John Paizis- Administrator -arrived.

The Adult Day Program is in a single-story commercial building, operating hours are 9:00 A.M. to 2:00 P.M. 5 days a week Monday thru Friday. All clients bring their own lunch or if clients forget their lunch the facility staff will provide emergency food out in the community.

At 11:15 AM, LPA Allen and John Paizis, toured the entire Adult Day Program inside and out which consisted of the following: Acting Classes Studio with a Green Room, Recording Studio, Post Production office, Administrative office, staff lounge/break room, multi-purpose room, lunch/lockers areas, Consumer Support office, four bathrooms, storage closet area, and a gated parking lot next door with 12 parking spaces that are rented by the ADP. Each room is adequately equipped and furnished. The hot water temperature measured at 113.1 degrees Fahrenheit, carbon monoxide and smoke detectors, fire extinguisher are fully charged, and are in working condition. The bathrooms are clean and operational, hazardous items are inaccessible to clients and first aid kit is fully stocked. The day program grounds are free of debris/hazards.

At 11:45 AM LPA Allen Also observed the programming schedules that are developed by the Director and Support Staff. The day program has a current activity schedule, and each instructor follows, dance class, music class, and acting classes monthly plans.
