Facility Evaluation Report

On 07/03/2025, Licensing Program Analyst (LPA) Pamela Bunker conducted an unannounced Case Management Visit to the above facility. LPA Bunker met with Eniola Ogunleye-Oni, DSP, and explained the purpose of the visit. LPA was granted entry to the facility. During today's visit, Licensee Brenda Chandler, Westside Regional Center, Service Coordinator Dwayne Coleman, and Program Manager Natasha Lopez also arrived.

DSP Eniola and LPA Bunker toured the facility's buildings and grounds to observe and identify any signs of neglect, abuse, or other immediate health and safety threats. No signs of neglect or abuse were observed during today's visit.

LPA Bunker requested and reviewed the facility's Emergency Disaster Plan, the Sleep Record, and the Fire Watch Record. LPA Bunker observed the facility to be clean, safe, sanitary, and in good repair. LPA Bunker observed that the facility was fully staffed at the time of the visit.

Licensee Brenda Chandler stated that the architect's estimate for the R3.1 has been received, and she will move forward with approving the plan.

An exit interview was conducted with DSP Eniola Ogunleye-Oni, and a copy of this report was given.
