Facility Evaluation Report
On 06/28/25 Licensing Program Analyst (LPA) Troy Watson conducted an unannounced case management visit to the above-named facility. LPA Watson met with the staff employee DSP Pearl Cash and explained the purpose of the visit. The purpose of the visit was to complete a health and safety check at the above-named facility.
On 06/29/25 LPA Watson requested and reviewed the facilities Emergency Disaster Plan, and the Sleep Record that is being used as a fire watch record. LPA Watson observed the facility clean and in good repair. LPA Watson observed facility adequately staffed at the time of visit.
An exit interview was conducted with the Licensee Brenda Chandler and a copy of this report was given.
