Facility Evaluation Report
On 07/14/2026 Licensing Program Analyst (LPA) Antonine Richard conducted an unannounced Required – 1 Year Inspection to the above-named facility and met with Administrator Kim Post. LPA explained the purpose of the visit and was accompanied by Staff Ogunleye-Oni toured the inside and outside the facility during this inspection. The facility is licensed to serve three (3) ambulatory, developmentally disabled adults ages 18 through 59 years only. The facility has an age exception for two clients in care.
The facility is a single-story family home located in a residential neighborhood that consists of, a living room, dining area, kitchen, TV/family room/office area, three (3) client bedrooms, two (2) bathrooms, a washer/dryer located in the detached garage and an indoor/outdoor activity area. There is outdoor shaded patio furniture.
Outside grounds were toured and no bodies of water were observed. Walkways around the home were clear of hazards. There are no security bars or weapons on the premises.
Three (3) out of three (3) client’s bedrooms were checked. Adequate lighting, plenty of dresser and closet space observed.
