Facility Evaluation Report
On Monday, June 15, 2026, Licensing Program Analyst (LPA) Pamela Bunker conducted an unannounced Case Management Visit to the above facility. LPA Bunker met with staff members, KoSarachi Ezeibe. LPA Bunker spoke with Licensee Brenda Chandler and Administrator Kim Post via telephone and explained the purpose of the visit. LPA was granted entry to the facility.
LPA Bunker and staff member KoSarachi Ezeibe toured the facility's buildings and grounds to observe and identify any signs of neglect, abuse, or other immediate health and safety threats. No signs of neglect or abuse were observed during today's visit.
LPA Bunker requested and reviewed the facility's Emergency Disaster Plan, the Sleep Record, and the Fire Watch Record. LPA Bunker observed that the facility was clean, safe, sanitary, and in good repair. LPA Bunker observed that the facility was fully staffed during the visit.
Ms. Chandler stated that Building and Safety approved the plans for the R3.1, and the Fire Department cleared the fire clearance. The facility received a new license to serve 3 non-ambulatory, developmentally disabled adults aged 18 through 59.
No deficiencies were cited during the visit.
