The inspector’s account

On 06/24/25, Licensing Program Manager (LPM) Ulysses Coronel and Licensing Program Analyst (LPA) Regina Cloyd conducted an unannounced case management – deficiency visit and met with Staff and explained the purpose of this visit. The purpose of this visit is to issue a deficiency observed on 06/20/25 and during today’s visit. On 06/20/2025, Licensing Program Analyst (LPA) Regina Cloyd conducted an unannounced Required – 1 Year Inspection to the above-named facility and met with Administrator Kim Post. The facility is licensed to serve three (3) ambulatory, developmentally disabled adults ages 18 through 59 years only. The facility has an age exception for two clients in care.

On 06/20/25, medical assessments for Clients 1 - 3 (C1, C2, and C3) indicated that clients are non-ambulatory. The facility license is for ambulatory clients. The Department’s record review of the Fire Safety Inspection Request by the Gardena Fire Department on 04/22/2005 indicates that the facility is only licensed to serve three ambulatory clients. The Department’s record review of the Application for Community Care Facility submitted by the Licensee on 03/09/2005 indicates a total capacity of three clients of which zero are non-ambulatory.

Deficiencies are being cited based on record review in accordance with the California Code of Regulations, Title 22, see LIC809D. An immediate civil penalty of $500 is issued.

An exit interview was conducted, plans of correction developed, and a copy of this report and appeal rights was discussed with Administrator Kim Post over the phone. A hard copy was left with Kosarachi Ezeibe.
