The inspector’s account

On Friday, November 07, 2025, Licensing Program Analyst (LPA) Pamela Bunker conducted an unannounced Case Management Visit to the above facility. LPA Bunker met with staff members, KoSarachi Ezeibe. LPA Bunker spoke with Licensee, Brenda Chandler via telephone and explained the purpose of the visit. LPA was granted entry to the facility.

LPA Bunker and staff member KoSarachi Ezeibe toured the facility's buildings and grounds to observe and identify any signs of neglect, abuse, or other immediate health and safety threats. No signs of neglect or abuse were observed during today's visit.

LPA Bunker requested and reviewed the facility's Emergency Disaster Plan, the Sleep Record, and the Fire Watch Record. LPA Bunker observed the facility to be clean, safe, sanitary, and in good repair. LPA Bunker observed that the facility was fully staffed at the time of the visit.

Ms. Chandler stated that Building and Safety will be coming out on Thursday, November 13, 2025. The plans for the R3.1 have been approved. Once the inspection is completed, the Fire Department will conduct its final review.

No deficiencies were cited during the visit.
